  • Patient ref
  • Payer
  • Exam
  • CPT

  • Status
  • Auth #
  • Auth expires
  • Days pending
  1. Open the Authorizations tab and replace the sample rows with your own — one row per entry. Formulas fill in automatically for new rows.
  2. Front-office use: patient references and auth numbers only; clinical notes go through the payer portal.
  3. Days pending turns amber after 5 days: escalate before the scheduled date. Auth expires turns red if the exam slips past it.
  4. Using Google Sheets? Upload the file to Google Drive, then open it with Google Sheets (or File → Import in an open sheet). Formulas, dropdowns and highlights carry over.

The file opens locally on your device — nothing is uploaded, and you own the copy outright.
